We are developing structures and methods to promote brain health and prevent memory disorders through multidisciplinary collaboration, supporting the well-being of ageing adults in the Kanta-Häme region.
A service pathway for the prevention of memory disorders is being developed in the region through multidisciplinary collaboration. The development of the pathway is based on the FINGER model, which enables healthcare professionals to implement preventive measures for memory disorders. The model includes identifying individuals at risk of memory disorders and supporting cognitive function through a comprehensive lifestyle program.
HAMK contributes research and evaluation expertise to the project. Students from HAMK’s social and health care degree programmes are also involved in the implementation.
Target group
Social and health care professionals from the Kanta-Häme wellbeing services county, municipal actors, and third sector organizations. Adults aged 65–75 living in Kanta-Häme and their close ones.
Operating area
The project’s primary geographical focus is the Kanta-Häme region, but through the research and development activities carried out within the project, its results and impact may extend more broadly at both national and international levels.
Goals
The goal is to develop structures and methods for the prevention of memory disorders to support the implementation of the FINGER model in the Kanta-Häme region.
In addition, the project aims to reduce the stigma associated with memory disorders, lower the threshold for discussing memory-related symptoms, and promote early identification of memory disorders.
Key actions
HAMK coordinates the development cycles that guide the creation of the service pathway.
- Development Cycle 1 – Disease Prevention and early detection.
- Development Cycle 2 – Integrating the FINGER model into regional services.
- Development Cycle 3 – Strengthening new competencies and embedding and evaluating the FINGER model.
